Steps:
All Permits issued will be based on the Biosafety Committee's assessment of laboratory "Biosafety Protocols."
Facility and Teaching labs will be issued a Permit based on the submitted protocols for a period up to five (5) years, with biennial compliance inspections required. Additional projects added within the Five years, will only be considered approved until the expiry of the Protocol Permit.
All new funded Research Projects require a new application to the Biosafety Committee before funding will be released by the Office of Research, in compliance with the Memorandum of Understanding between the University and the Tri-Council Agencies.
Renewal / Amendments will fall into one of the following categories:
Annual Renewals:
1) Renewal with No Changes from the existing permit.
Procedure:
i) Permit Holder will complete the "Renewal" section of the renewal/amendment form.
2) Renewal with addition of new personnel.
i) Permit Holder will complete the "Renewal" section of the renewal/amendment form.
ii) Permit Holder will complete and submit the Committee a record of training for each new lab personnel.
3) Renewal with addition of organism at the same Risk Group level as already approved.
i) Permit Holder will complete the "Renewal" section of the renewal/amendment form.
ii) Permit Holder will provide information on the new organism, including risk group level and commitment that no new
experimental procedures (in addition to those listed on the approved Biological Risk Assessment form) will be undertaken
with this organism.
Amendments:
1) Amendment with addition of new personnel.
i) Permit Holder will complete the "Amendment" section of the renewal/amendment form.
ii) Permit Holder will complete and submit the Committee a record of training for each new lab personnel.
2)
Amendment with addition of biohazard at the same level as currently approved.
i) Permit Holder will complete the "Amendment" section of the renewal/amendment form.
ii) Permit Holder will provide information on the new organism, including risk group level and commitment that no new
experimental procedures (in addition to those listed on the approved Biological Risk Assessment form) will be undertaken
with this organism.
Requiring New Application to the Committee:
1) New Funded Research Project
i) If there are no changes in Risk Group of biohazard, and no changes to the procedures as approved on the Biological
Risk Assessment Form, the Permit Holder files a new "research" application to the Committee completing sections
1,2,3,4 and 12 of the form.
2)
New Funded Research Project
i) If there are changes in Risk Group of biohazard, OR changes to the procedures as approved on the Biological
Risk Assessment Form, the Permit Holder files a new full "research" application to the Committee completing all sections
of the application form. The Permit Holder must submit a revised Biological Risk Assessment and Revised Laboratory
Biosafety Manual.
3) Work at an increased level from which Permit Holder is currently approved.
i) If there is an increase in Risk Group of biohazard, OR changes to the procedures as approved on the Biological
Risk Assessment Form, the Permit Holder files a new full application to the Committee completing all sections
of the application form. The Permit Holder must submit a revised Biological Risk Assessment and Revised Laboratory
Biosafety Manual.
